MAC Sports Waiver and Release of Liability Agreement
In consideration of being permitted to participate in a MAC Sports program, practice, training session, camp, game, tournament, trip, or related activity (each an “Event”), I acknowledge and agree to the following on behalf of myself or, if I am signing for a minor, on behalf of the minor participant identified below.
1. Acknowledgment of risk. Hockey and other sports activities carry risks of injury and illness. These include collisions, falls, equipment-related injuries, serious or permanent injury, paralysis, death, and exposure to communicable illness. Rules, supervision, protective equipment, and careful conduct can reduce some risks but cannot eliminate them.
2. Assumption of risk. I understand these risks and voluntarily accept the known and unknown risks associated with participation in an Event, including risks arising from the acts or omissions of other participants or the parties identified in Section 4, to the extent permitted by applicable law.
3. Participant conduct. The participant will follow applicable Event rules and the reasonable directions of coaches, officials, and staff. If I become aware of a significant hazard, I will stop participation when appropriate and promptly notify an Event official.
4. Release and indemnification. To the fullest extent permitted by applicable law, I release and agree to hold harmless Mac Sports Inc., doing business as MAC Sports, its owners, directors, officers, coaches, staff, volunteers, agents, sponsors, Event organizers, other participants, and the owners and operators of Event facilities (collectively, the “Released Parties”) from claims arising out of the participant’s involvement in an Event, including claims for injury, illness, death, or property loss allegedly caused by a Released Party’s ordinary negligence. To the fullest extent permitted by applicable law, I also agree to indemnify the Released Parties against claims arising from the participant’s conduct or my breach of this agreement. Nothing in this agreement releases a claim that cannot legally be released.
5. Photo, video, and audio permission. I permit MAC Sports to photograph, film, livestream, and record the participant during Events and to use those recordings, the participant’s image, and the participant’s voice in its Event coverage, website, social media, and promotional materials without compensation.
6. Governing sports organization. If an Event requires USA Hockey, AAU Hockey, or another governing-body registration, I confirm that the participant holds the registration required for that Event and will comply with the applicable rules.
7. Duration and acknowledgment. This agreement applies to the participant’s involvement in Mac Sports Events. I have read it, understand that it affects substantial legal rights, and sign it voluntarily.
If the participant is a minor: I certify that I am the participant’s parent or legal guardian, have authority to sign on the participant’s behalf, and consent to the participant’s involvement in the Event.
